Sublingual is literally means “under the tongue”. This refers to the pharmacological route of administration by which drugs diffuse into the blood through tissues under the tongue. Several drugs nowadays are designed for sublingual administration particularly cardiovascular drugs for faster onset of action.

Sublingual drug administration has certain advantage over oral administration. Being more direct, it has rapid absorption rate and faster onset of action. This is because of the lack of a stratum corneum epidermis and the rich vascular supply of the mucosa, wherein the drug diffuses into the capillaries and enters the venous circulation until it reaches the targeted organs and body system. It has the advantage of avoiding the enterohepatic circulation and immediate destruction by gastric acid or partial first-pass effects of hepatic metabolism. For significant drug absorption to occur across the oral mucosa, the drug must have a prolonged exposure to the mucosal surface.
